How to Order your
SafeVision X-Ray Glasses
1) Select your frame from below and remember the
style name and number.
2) Print out our orderform and complete all
lines.
3) Fax your orderform and current eyeglass prescription to us at
314.918.0057.
(If ordering Plano, no need to send in an eyeglass prescription, just fax
the orderform)
4) If you are a bifocal wearer, please call us
toll free at 888-254-7406 to discuss your bifocal measurement requirements
prior to faxing your completed orderform.
Note: If you are ordering
single vision or bifocal prescription lenses, we must have a copy
of your current eyeglass prescription on your Doctor's form in order to
make your glasses. Your PD (Pupil Distance) must be on the
prescription in order for us to make your glasses. If your PD is not
on the prescription you have, call your Doctor or eyeglass vendor, write
the measurement on your prescription, and then fax to us. If you are
ordering plano lenses (non prescription) you don't need to include a
prescription.
