Step 1. Obtain your current eyeglass prescription from your eye care professional. The prescription must be on your Doctor’s stationary or form, with your Pupil Distance (PD) measurement listed.  Your pupil distance measurement is required for all orders so please ensure it is on your prescription before sending your order for processing.

Please provide an actual copy of your prescription on your Doctor's letterhead so we can see any comments noted by your Doctor.

Step 2. Select your Safety Frame

Step 3. Print out and complete the order form.

Step 4. Send your completed order form and prescription copy to
